Southwest Care—an FQHC serving the Santa Fe and Albuquerque communities—was struggling with inconsistent documentation, coding, referral management, refill workflows, prior authorizations, and document handling across its clinics. To address these gaps, Southwest Care partnered with Onpoint Healthcare Partners for their Iris AI Agent Platform—which includes ChartFlow, CodeFlow, CareFlow, and NetworkFlow—and related services. Following implementation, the organization reports reduced administrative burden, more consistent documentation and workflow execution, increased provider capacity for patient care, and financial gains from improved documentation, consistency of workflow execution, and revenue capture.
