Southwest Care—an FQHC serving the Santa Fe and Albuquerque communities—was struggling with inconsistent documentation, coding, referral management, refill workflows, prior authorizations, and document handling across its clinics. These challenges created a heavy administrative burden for clinicians, contributed to burnout risk, and left the organization vulnerable to missed revenue. To address these gaps, Southwest Care partnered with Onpoint Healthcare Partners for their Iris AI Agent Platform—which includes ChartFlow, CodeFlow, CareFlow, and NetworkFlow—and related services. Following implementation, the organization reports reduced administrative burden, more consistent documentation and workflow execution, increased provider capacity for patient care, and financial gains from improved documentation, consistency of workflow execution, and revenue capture. Because a meaningful portion of the ROI is tied to services and workflow standardization—not solely to software automation—this report evaluates the combined impact of Onpoint’s platform and services.
