ADT Notifications

Technology whose primary function is to enable admission, discharge, and transfer (ADT) notifications between inpatient facilities and a patient's primary care physician, care manager, or other key healthcare providers (such as medical specialists, physician groups, ACOs, and FQHCs) with the goal of ensuring proper care coordination and follow-up and preventing unnecessary hospital readmissions. Also triggers broader care coordination functionality, including workflows, insights, and optimizations, for various parties with vested interest in the patient's care.
