At its introduction, the electronic medical record (EMR) provoked a ground swell of large-scale EMR implementations, mostly dominated by five professional services firms (PSFs): Accenture, ACS, CTG, FCG, and IBM. Now the bulk of EMR implementations—large projects, big budgets, major firms—is nearly gone. The market landscape is different, with smaller clients, more limited projects, and a multitude of firms. Acquisitions have diffused talent across many smaller firms, who are taking a bite of the pie once owned by a select few.
