Innovative Care Management Program Used to Cut Costs and Transform Lives of Super-Utilizers
To better serve individuals experiencing homelessness—who are often super-utilizers of The Queen’s Health Systems’ inpatient and emergency department (ED) services—the stakeholders in this case study worked together to enable the healthcare organization to first identify these individuals and then provide community health workers to help connect them with available community resources. The resulting program has been extremely successful for all parties involved—first and foremost for the patients and the clinicians who serve them.
