Xealth’s digital health platform helps organizations to proactively educate, monitor, and connect with patients during COVID-19, and enables them for a future in digital care delivery.

Xealth, the leading platform for connecting digital health, gives clinicians and patients easy access to critical digital health tools. The Xealth platform is embedded in the EHR, so clinicians can prescribe and monitor digital health content, apps, devices, and services from within their charting workflow. Xealth can deliver these assets to the patient via email or through the health system’s existing patient portal, driving engagement and increasing adoption.

In response to the spike in telehealth visits, we are powering patient education on getting set up for these visits and troubleshoot technical issues.

Remote patient monitoring

Two health systems are using a care pathway solution to immediately enroll patients who are symptomatic into a remote monitoring pathway so that even though they are quarantined at home, they have the ability to be monitored in real time by a virtual nursing team.

There's currently a massive influx of patients that are stressed, anxious, or depressed, and no facilities to treat them in traditional settings. One system is rolling out a digital mental health tool across three states.

Once Xealth is deployed, these programs can be deployed in between five and 14 days.
