The Quest for Patient Safety
The driving force behind smart pumps has always been greater patient safety. Differences exist in how well various pumps help with this goal, so KLAS turned to the end users—mainly nurses—to gather insights regarding drug-library compliance, reporting, ease of use, and wireless functionality. BD is gauged to have the most comprehensive pump platform, while Baxter leads in ease of use and drug-library compliance for LVPs. Pfizer is rolling out new pumps. Both B. Braun and Smiths Medical lag behind in wireless adoption and integration.
1. BAXTER CUSTOMERS ACHIEVING THE HIGHEST DRUG-LIBRARY COMPLIANCE
Evident in their high levels of drug-library compliance, Baxter Healthcare’s LVP users feel the pump’s strong ease of use improves patient safety and enables quick access to the drug library. A large number of BD customers also achieve high compliance, which many credit to BD’s reports and guidance. Unlike Pfizer’s newer pumps, the Plum A+ does not automatically direct users to the library, resulting in lower compliance levels. The drug-library functionality and navigation in B. Braun’s Infusomat Space do not surpass that of their older Outlook ES; issues exist with drug-library updates, boluses, and piggybacks.
2. BD—FOLLOWED BY PFIZER—LEADS IN CRITICAL AREA OF EMR INTEGRATION
Patient safety requires more than drug-library compliance—bidirectional EMR integration is named by providers as the most important next step for improving safety. Such integration eliminates pump-programming errors and is a top criteria in today’s purchasing decisions. BD’s Alaris has the most customers live with integration, is the most purchased pump, and is multimodular. Pfizer’s Plum A+ has the second highest number of integrated customers. Baxter’s pumps are highly functional but lack bidirectional EMR integration. B. Braun has glitchy software and inconsistent support.
3. BD’S REPORTS MORE ACTIONABLE; ON VERGE OF GOING BEYOND COMPLIANCE
Providers want reports that give them actionable compliance data, such as which users bypass the drug library and which pumps and infusion types are problematic. No vendor today consistently delivers this. Most BD customers report receiving helpful compliance reports, and a limited number are starting to also get more data and actionable insights—at additional cost—that help reduce alarm fatigue. Pfizer customers find their reports less actionable. Under 60% of Baxter respondents say their reports help with compliance. Fewer B. Braun or Smiths Medical customers regularly get reports as many have yet to deploy wireless functionality.
4. PCA: SMITHS MEDICAL LAGGING IN DEVELOPMENT AND SUPPORT
The Smiths Medical CADD-Solis is the only PCA pump in this report used in both ambulatory and acute care settings. Hospitals say that while the pump has positive features, users don’t get the post-go-live support, wireless capabilities, or reports expected in acute care. Pfizer and BD customers receive help with the initial setup as well as continued guidance. Pfizer customers have seen improvements to the reliability, quality, and safety of the PCA pump; many BD customers like that the LVP, PCA, and syringe modules are on the same system.
5. SYRINGE: SMITHS MEDICAL'S RELIABILITY DOES NOT OUTWEIGH THE DRAW OF BD'S INTEGRATION
Smiths Medical customers have high comfort and compliance levels with the relatively small drug library needed for pediatrics. However, some are switching to BD to gain bidirectional EMR integration and a wireless, consolidated platform. Smiths Medical has neither validated EMR integration nor a consolidated platform; wireless has been available for at least four years, but KLAS has yet to validate many customers using it.
