Unite Us
Emerging Technology Spotlight 2020
Solutions for social determinants of health (SDOH) are a top-of-mind subject for healthcare decision makers. SDOH vendors work to extend the care of patients beyond the walls of the hospital or clinic to address all the factors that make up a healthy population, including food insecurity, inadequate housing, mental health care, etc. Unite Us differentiates themselves by building robust networks of health and social services in the markets they serve. To date, they have built 55 of these community-based networks and have 40 additional networks in process. This spotlight validates the early outcomes and performance of Unite Us.
