Moving Closer to Documentation Completion

Providers approach clinical documentation improvement (CDI) in multiple ways, but all are looking to improve the same things: their physician adoption, clinical documentation specialist (CDS) productivity, chart completeness and accuracy, and case mix index (CMI). Some turn to vendors for software and workflow tools to help them manage queries and reporting; some engage consulting firms for implementations, optimization, or specialized CDS and physician training; and yet others want a one-stop CDI shop that does everything.

1. REVENUE CYCLE TRANSFORMATION FIRMS VS. CAC+CDI FIRMS

Due to strong training and expertise, clients of consulting firms like Huron, Navigant, and PwC report a higher impact on CDI improvement than do clients of vendors providing both CAC and CDI technology (such as 3M, Optum360, and Precyse). Providers feel that ICD-10 and the subsequent increase in demand for tools and services have led CAC+CDI vendors to release products that haven’t been fully baked and have resulted in implementation and ongoing support resources becoming stretched. Since many providers plan to combine CAC and CDI, there is a lot of pressure in the market for CAC+CDI vendors to address these gaps.

2. EXPERTISE, SERVICES FROM PWC, NUANCE, AND NAVIGANT DRIVE STRONG RESULTS

PwC’s strong physician training has earned them the highest score for the impact on physician behavior and CDS productivity, though some customers mention pricing concerns. Navigant customers praise the firm’s thorough training; many, though, feel Navigant’s tools need fine-tuning. Nuance’s CDI services have driven lasting physician behavior improvements and CDS productivity for most clients in this report, and Nuance is the only vendor in this report who has proven to be able to consistently deliver a high impact across a large client base. Some clients do complain of underdeveloped tools.

3. CHARTWISE ACHIEVES IMPACT WITH INTUITIVE TOOLS: 3M AND PRECYSE STRUGGLE TO DELIVER CONSISTENTLY

ChartWise’s cloud-based CDI tools have a high impact among clients, who say the tools are stable and easy to use. They also report strong hands-on support at every level, from the CEO down. 3M and Optum360 clients say these vendors’ strengths are the tools and development. Though they mention some issues in these areas, they say the biggest gaps are in the implementations and ongoing support. Customers of newer entrant Precyse say the CDI tools have improved and that Precyse provides strong, responsive communication and interaction while continuing to develop the tools.
